Home and Community Based Services

Solutions provides services for:

  • Behavioral management problems

  • Oppositional and defiant behavior

  • Social immaturity

  • Aggressive behavior

  • Affective instability

  • Impulsivity

  • Conduct problems

  • Self-injurious behavior

Solutions is licensed under Minnesota Corporate Foster Care regulations to provide 24-hour Home and Community-Based services (HCBS). These are secured through a county service agreement and are funded through a variety of waiver programs. Solutions' HCBS target those individuals who have had a history of disrupted placement due to serious mental health or behavioral difficulty. The focus is on maintaining placement stability and promoting independence and community integration.

  • Psychiatric consultation and medication management

  • Rehabilitative mental health services (such as CTSS and ARMHS)

  • Risk management assessment

  • Diagnostic assessments

  • Comprehensive behavioral assessment intervention

  • Functional skill assessment

  • Individual, group and family therapy

  • Mental health care management

  • 24-hour crisis intervention

Corporate Foster Care

Programming is designed to target:

  • High behavioral management needs

  • Those with interrupted placement histories

  • Those needing crisis on-call support

Solutions provides 24-hour care in individual and small group community settings and is licensed under foster care regulations. Arrangements are generally made through a service agreement with the county case manager.

 

Independent Living Services (ILS)

Independent living skills training is provided through a service agreement by the county case manager. Training focuses on building skills and managing barriers to successful community placement. ILS can be used as a bridge to step down from corporate foster care in order to maximize community independence. Solutions actively works to ensure the least restrictive placement setting possible.
